6.1.2
All lifts shall be periodically inspected, maintained, tested (performance-verification), and documented at intervals determined by the manufacturer or a qualified testing body. This shall be performed at least once a year. Documentation of the periodic inspection shall include but is not limited to
a) name and address of the manufacturer;
b) who is completing the testing;
c) brand;
d) date of test;
e) test weight;
f) location (room number);
g) serial number or equivalent identification;
h) results of all items inspected, maintained, and tested;
i) pass or fail; and
j) any action required.
